Safety metrics do not fail. What fails is the way they are used.

At Cotesa, reading the metrics without filters and without window dressing led us to a significant decision: to bring safety closer to the reality of the field.

The data did not become a report. It became an operational decision.

We built a system that combines managerial accountability, lagging and leading indicators such as the safety performance index, and an active presence in the operation, connecting culture, process and execution.

That effort gave rise to PSEP, our Safety, Efficiency and People Programme, and sustained 12 months with no lost-time accidents.

One lesson became clear along the way: which metrics you choose depends on where the organisation stands.

In many cases, progress starts with the basics done well, with metrics such as the severity rate, which make the real impact of events clear and add consistency to the analysis.

From there, culture stops being talk and starts reinforcing a system that already works in practice.

In the end, it does not matter how many metrics you have, but what you do when they show you something you would rather not see.
